چکیده

This paper is a review of physiological and behavioral data on motor cortex stimulation (MCS) and its role in persistent neuropathic pain. MCS has been widely used in clinical medicine as a tool for the management of pain that does not respond satisfactorily to any kind of conventional analgesia. Some important mechanisms involved in nociceptive modulation still remains unclear. The aim of this study was to describe the mechanisms involved in neuropathic pain and introduce the effectiveness of electrical stimulation of the motor cortex used in the treatment of this disease. The ascending pain pathways are activated by peripheral receptors, in which

Effects of Cortical and Peripheral Electrical Stimulation on Brain Activity in Individuals with Chronic Low Back Pain

Purpose: Neuroscience studies suggest that Chronic Low Back Pain (CLBP) is associated with central sensitization, and maladaptive reorganization of the brain; this introduced a new target for LBP treatment. Studies revealed that cortical and peripheral electrical stimulation can be beneficial in regulating brain neuronal activity. However, there is a scarcity of evidence to support the effects ...

Event-related cortical processing in neuropathic pain under long-term spinal cord stimulation.

BACKGROUND Several mechanisms were suggested in the past to explain the beneficial effect of spinal cord stimulation (SCS) in patients suffering from neuropathic pain. Little is known about potential supraspinal mechanisms. OBJECTIVE In this study cortical signaling of patients with neuropathic pain and successful long-term treatment with SCS was analyzed. STUDY DESIGN Observational study. ...

Somatotopic organization of the analgesic effects of motor cortex rTMS in neuropathic pain.

BACKGROUND Motor cortex repetitive transcranial magnetic stimulation (rTMS) was found to relieve chronic neuropathic pain, but the optimal parameters of stimulation remain to be determined, including the site of stimulation. OBJECTIVE To determine the relationship between cortical stimulation site and pain site regarding the analgesic efficacy of rTMS of motor cortex in chronic neuropathic pa...

Non-invasive transcranial direct current stimulation for the study and treatment of neuropathic pain.

In the last decade, radiological neuroimaging techniques have enhanced the study of mechanisms involved in the development and maintenance of neuropathic pain. Recent findings suggest that neuropathic pain in certain pain syndromes (e.g., complex regional pain syndrome/reflex sympathic dystrophy, phantom-limb pain) is associated with a functional reorganization and hyperexitability of the somat...

A pilot study investigating the effects of fast left prefrontal rTMS on chronic neuropathic pain.

OBJECTIVE Stimulating the human cortex using transcranial magnetic stimulation (TMS) temporarily reduces clinical and experimental pain; however, it is unclear which cortical targets are the most effective. The motor cortex has been a popular target for managing neuropathic pain, while the prefrontal cortex has been investigated for an array of nociceptive pain conditions. It is unclear whether...
